Is this a good deal? The shipping from Italy to the US is a Deal Breaker for me, but I am curious as I am not familiar with this Blood release.

That seller, The PC_Gamer is a pretty nice guy I think. Not exactly the least expensive, but a good stable seller for years in my opinion. I have bought several things from them over the years and they always answer all my questions I ask about boxes and condition etc.

Yeah ... normaly some games are "indiziert" so its only not allowed to sell these games ... but "beschlagnahmt" is a kind of worse ... someone wrote me a message with that information (i never heard about that) ...

Ah, the notorious German index of games, in other words, the halls of the damned. Any videogame ending up there is completely illegal for selling/owning/whatever. Nice to see Doom getting released from the abyss of eternal peril of games.
